Consent to Participate
This document serves as a comprehensive consent, acknowledgement, and liability waiver for the use of the Rejuven8 Wellness Club’s facilities, services, and offerings. By signing this form, you confirm that you have read, understood, and agreed to the terms outlined below regarding participation in any activities, therapies, consultations, or treatments conducted on-site.
I, the undersigned, voluntarily agree to participate in services and/or activities offered at Rejuven8 Wellness Club. I acknowledge that Rejuven8 provides access to wellness, recovery, and health optimisation services that may include, but are not limited to:
• Infrared sauna, ice bath, hyperbaric oxygen therapy (HBOT), Normatec compression
• IV drips, diagnostics, physiotherapy, chiropractic care
• Assisted stretching, light therapy, aesthetic treatments
• Group or individual wellness consultations
• Use of gym recovery tools and equipment
• Consumption of food or beverages from the Fuel Bar
I understand that participation in such services involves inherent risks and I confirm that I am participating voluntarily and at my own discretion.
Independent Medical Services Disclosure
I acknowledge that Rejuven8 Wellness Club is not a DHA-licensed medical facility. Any medical-related services (e.g. IV therapy, physiotherapy, chiropractic treatment) are delivered by licensed freelance professionals, operating independently under their own DHA registration. Rejuven8 provides space and coordination but is not the medical provider of record.
I understand that:
• All consultations and procedures are to be conducted under the discretion and responsibility of the licensed practitioner.
• I am encouraged to ask questions and seek clarification before consenting to any service.
• I have the right to refuse any treatment or procedure at any time.
Medical Fitness & Disclosure
I confirm that:
• I am in good general health and do not suffer from any condition that may prevent me from safely using the services or equipment at Rejuven8.
• I have disclosed all relevant medical conditions, allergies, injuries, or sensitivities to staff or practitioners before treatment.
• I understand that it is my responsibility to update the facility on any changes to my health status prior to future visits.
Assumption of Risk & Release of Liability
In consideration of being permitted to use the facilities and services of Rejuven8, I hereby:
• Waive and release Rejuven8 Wellness Club, its employees, affiliates, owners, and partners from any and all claims, liabilities, or demands of any kind arising from participation in services, except in cases of proven gross negligence.
• Accept full responsibility for my own health, decisions, and wellbeing while at the facility.
• Agree not to hold Rejuven8 liable for the independent actions of freelance practitioners providing services on-site.
